How to use this template

5S is a method for organising the workplace so that waste and problems become visible. This audit measures how well each of the five S's is anchored — not to give marks, but to see where you stand and what to improve next time.

When do you use it?

At a fixed interval — weekly or monthly per zone. 5S only works if the audit is repeated; a one-off clean-up always falls back.

Time & participants

15-30 min per zone. Audit together with an employee from the area.

Step by step

  1. Pick one zone at a time.
  2. Go through the criteria and score each 1-5 based on what you actually see.
  3. Note concrete deviations — not just the number.
  4. Agree an action, owner and deadline for the lowest scores.
  5. Compare with the last audit to see the trend.
  6. Make the score visible in the area so the team owns it.

Watch out

  • Score on what you see now — not on "usually".
  • A high score without a visible standard is not real 5S.
  • The fifth S (self-discipline) is the hardest — measure whether the standards hold without a reminder.

Example

Seiton (set in order) scores high when every item has a marked place, and a missing item is seen at once — e.g. shadow boards for tools.
